ABSTRACT

RESUMO Objetivo: Estimar a prevalência de burnout em médicos intensivistas que trabalham em unidades de terapia intensiva adulto, pediátrica e neonatal, de cinco capitais brasileiras. Métodos: Estudo epidemiológico descritivo, com amostra aleatória e estratificada por conglomerado, de 180 médicos intensivistas de cinco capitais, representando as regiões geográficas brasileiras: Porto Alegre (RS), São Paulo (SP), Salvador (BA), Goiânia (GO) e Belém (PA). Um questionário autoaplicável avaliou dados sociodemográficos e o nível de burnout foi avaliado por meio do Maslach Burnout Inventory. Resultados: Foram avaliados 180 médicos, sendo 54,4% do sexo feminino. A média de idade foi 39 ± 8,1 anos, 63,4% com a especialização como a maior titulação, 55,7% com até 10 anos de trabalho em unidade de terapia intensiva e 46,1% possuíam título de especialista em terapia intensiva. A maioria (50,3%) tinha carga horária semanal de trabalho entre 49 e 72 horas, e o tipo de vínculo mais frequente foi empregado assalariado. Níveis elevados de exaustão emocional, despersonalização e ineficácia foram encontrados em 50,6%, 26,1% e 15,0%, respectivamente. A prevalência de burnout foi de 61,7%, quando considerado nível alto em pelo menos uma dimensão e de 5% com nível alto nas três dimensões simultaneamente. Conclusão: Observou-se elevada prevalência da síndrome de burnout entre os médicos intensivistas. Estratégias para promoção e proteção à saúde desses trabalhadores devem ser discutidas e implementadas nos hospitais.


ABSTRACT Objective: To estimate the prevalence of burnout in intensivist doctors working in adult, pediatric and neonatal intensive care units in five Brazilian capitals. Methods: Descriptive epidemiological study with a random sample stratified by conglomerate with 180 intensivist doctors from five capitals representing the Brazilian geographic regions: Porto Alegre (RS), Sao Paulo (SP), Salvador (BA), Goiania (GO) and Belem (PA). A self-administered questionnaire examining sociodemographic data and the level of burnout was evaluated through the Maslach Burnout Inventory. Results: A total of 180 doctors were evaluated, of which 54.4% were female. The average age was 39 ± 8.1 years, 63.4% had specialization as the highest degree, 55.7% had up to 10 years of work experience in an intensive care unit, and 46.1% had the title intensive care specialist. Most (50.3%) had weekly workloads between 49 and 72 hours, and the most frequent employee type was salaried. High levels of emotional exhaustion, depersonalization and inefficacy were found (50.6%, 26.1% and 15.0%, respectively). The prevalence of burnout was 61.7% when considering a high level in at least one dimension and 5% with a high level in three dimensions simultaneously. Conclusion: A high prevalence of burnout syndrome among intensivist doctors was observed. Strategies for the promotion and protection of health in these workers must be discussed and implemented in hospitals.

ABSTRACT

Para evaluar la confiabilidad y precisión de la información médica proporcionada por los medios de comunicación en Argentina, alumnos avanzados de medicina, que desconocían los objetivos del estudio, identificaron aseveraciones relacionadas a temas médicos transmitidas por medios de comunicación. Los hallazgos fueron contrastados con recomendaciones realizadas por médicos expertos en la toma de decisiones basada en evidencias. Las recomendaciones de los medios y las confeccionadas por los expertos fueron comparadas en relación a su fuerza y dirección. Se identificaron 81 recomendaciones/preguntas las que fueron contestadas por los expertos, 15 con alta, 18 con moderada, 30 con baja y 18 con muy baja calidad de evidencia. Solamente el 53% (IC95% 42-64%) de las recomendaciones hechas por los medios de comunicación coincidieron en la dirección (a favor o en contra de la intervención) con las realizadas por los expertos y el 28% (IC95% 18-39%) fueron calificadas como inadecuadas (diferencias significativas tanto en dirección como en fuerza). El análisis del subgrupo de recomendaciones realizadas en los medios por profesionales de la salud mostró una coincidencia en la dirección del 71% (IC95% 56-86%) con 17% (IC95% 6-33%) de recomendaciones inadecuadas, OR = 0.35 (IC95% 0.1-1.1) en relación a las no realizadas por profesionales de la salud. Se concluye que la información médica que proveen los medios de comunicación en Argentina es poco confiable, lo que posiblemente tenga un impacto negativo sobre el funcionamiento del sistema de salud y la relación de los médicos con sus pacientes.


To evaluate the certainty and accuracy of the healthcare information provided by the mass media in Argentina, a group of senior medical students, blind to the study objectives, identified healthcare related statements transmitted through mass media. These findings were challenged against the recommendations of a group of physicians trained in evidence-based decision making (EBDM). We compared the strength and direction of the mass media recommendations with those of experts on EBDM. Eighty one recommendations/questions were identified and answered by the experts on EBDM, 15 with high, 18 with moderate, 30 with low and 18 with very low quality of evidence. Only 53% (CI95% 42-64%) of the mass media recommendations agreed with the expert recommendation in direction (for or against) and 28% (CI95% 18-39%) were classified as inappropriate (significant discrepancies both in direction and strength). Subgroup analysis revealed that 71% (CI95% 56-86%) of there commendations made by professionals in mass media agreed with experts in direction and 17% (IC95% 6-33%) were classified as inappropriate, OR = 0.35 (CI95% 0.1-1.1) compared to recommendations in mass media by non-professionals. We conclude that the healthcare information provided by mass media in Argentina is unreliable; this fact can probably have a negative impact in the health system performance and physician-patient relationship.

ABSTRACT

The power of interpretation in the analysis of cranial computed tomography (CCT) among neurosurgeons and radiologists has rarely been studied. This study aimed to assess the rate of agreement in the interpretation of CCTs between neurosurgeons and a radiologist in an emergency department. Method 227 CCT were independently analyzed by two neurosurgeons (NS1 and NS2) and a radiologist (RAD). The level of agreement in interpreting the examination was studied. Results The Kappa values obtained between NS1 and NS2 and RAD were considered nearly perfect and substantial agreement. The highest levels of agreement when evaluating abnormalities were observed in the identification of tumors, hydrocephalus and intracranial hematomas. The worst levels of agreement were observed for leukoaraiosis and reduced brain volume. Conclusions For diseases in which the emergency room procedure must be determined, agreement in the interpretation of CCTs between the radiologist and neurosurgeons was satisfactory. .


O poder de interpretação na análise de tomografias de crânio (TCC) entre neurocirurgiões e radiologistas tem sido pouco estudada. O objetivo deste estudo é avaliar as taxas de concordância na interpretação de TCCs entre neurocirugiões e radiologista em um departamento de emergência. Método 227 TCCs foram independentemente analizadas por 2 neurocirugiões (NC1 e NC2) e um radiologista(RAD). O índice de concordância nas análises foi estudada posteriormente. Resultados O valor de Kappa obtido entre os NC1 e NC 2 e entre estes e RAD foram quase perfeitos e substancial respectivamente. O maiores índices de concordância quando avaliadas anormalidades foram observados na identificação de tumores, hidrocefalia e hematomas intracranianos. O piores índices foram observados com relação a leucaraiose e redução volumétrica. Conclusão Para doenças apresentadas em um departamento emergência que demandam tratamento mais agressivo o índice de concordância na interpretação de TCCs entre RAD e NC foi satisfatório. .

ABSTRACT

A 52-year-old woman undergoing azathioprine treatment for rheumatoid arthritis developed acute retinal necrosis a month after intravitreal dexamethasone (Ozurdex ®) implantation for posterior uveitis in the left eye. Varicella zoster virus (VZV) DNA was detected in the anterior chamber and vitreous samples on polymerase chain reaction (PCR) analysis. Retinal detachment occurred despite systemic and intravitreal antiviral therapy. Favorable structural and functional outcomes were achieved after retinal surgery with silicone oil. To the authors’ knowledge, this is the first reported case of acute retinal necrosis following placement of an Ozurdex® implant. Physicians practicing Ozurdex® implantations should be aware of this unusual but devastating complication. Extra caution and frequent follow-up are required in all immunocompromised patients receiving Ozurdex® implantation.


Uma mulher de idade de 52 anos em tratamento azatioprina para a artrite reumatóide desenvolveu necrose aguda de retina um mês após implantação Ozurdex® para uveíte posterior do olho esquerdo. DNA de varicela zoster (VZV) foi detectado em amostras de câmara anterior e vítreo por análise de PCR. Apesar da terapia antiviral sistêmica e intravítrea, o paciente apresentou descolamento de retina. Desfecho favorável estrutural e funcional foi obtida após a cirurgia retiniana com óleo de silicone. Pelo conhecimento dos autores, este é o primeiro caso relatado de necrose aguda de retina após a colocação de um implante Ozurdex®. Os médicos que implantam Ozurdex® devem estar cientes desta complicação incomum, mas devastadora. É necessário cuidado extra e acompanhamento frequente dos pacientes que recebam o implante Ozurdex® e apresentem qualquer condição imunocomprometedora.

Background: A tight glycemic control of hospitalized patients increases the risk of hypoglycemia, whose management is not always optimal. Aim: To assess the hypoglycemia management competences of a multidisciplinary team in a clinical hospital. Material and Methods: An anonymous questionnaire about hypoglycemia management was answered by 11 staff physicians, 42 residents and 28 nurses of the department of medicine and critical care unit ofa university hospital. Results: Respondents had a mean of 60 percent of correct answers, without significant differences between groups. The capillary blood glucose level that defines hypoglycemia was known by most of the respondents, but the value that defines severe episodes was known only by 60 percent. The initial management and follow up was well known only for severe episodes. Less than 50 percento knew the blood glucose value that required continuing with treatment. Conclusions: Although most professionals are able to recognize hypoglycemia, the knowledge about is management if insufficient.

CONTEXT AND OBJECTIVE: Infant hearing deficiency is a human disorder with devastating effects and serious implications for the development of speech and language. Early diagnosis of hearing loss should be the objective of a multidisciplinary team, and early-intervention programs should immediately follow this. The aim of this study was to investigate the knowledge and conduct of pediatricians and pediatric residents in a tertiary teaching hospital regarding deafness. DESIGN AND SETTING: Cross-sectional study in a tertiary hospital in the state of São Paulo, Brazil. METHODS: Eighty-eight questionnaires were randomly distributed to pediatricians and pediatric residents. RESULTS: Thirty-six questionnaires were analyzed. Most respondents (61.1 percent) were residents in pediatrics and/or neonatology. Eighty-three percent of them performed special procedures on babies presenting a high risk of deafness, and 55 percent reported that they had no knowledge of techniques for screening hearing. Most of them were unaware of the classifications of level and type of hearing loss. According to 47.2 percent of them, infants could begin to use a hearing aid at six months of age. Most of them reported that infants could undergo hearing rehabilitation during the first six months of life, and all respondents stated 's responsibilities. CONCLUSIONS: Even though most of the participants followed special procedures with babies presenting a high risk of deafness, they did not routinely investigate hearing. All respondents believed that it is a doctor's responsibility to be concerned about child communication.


CONTEXTO E OBJETIVO: A deficiência auditiva na criança é uma das desordens humanas cujo impacto tem efeito devastador produzindo sérias conseqüências no desenvolvimento da fala e linguagem. O diagnóstico precoce da perda auditiva deve ser objetivo de uma equipe interdisciplinar e ser seguido imediatamente por programas de intervenção precoce. O objetivo foi investigar o conhecimento e condutas de pediatras e residentes em pediatria de um hospital-escola terciário em relação à surdez. TIPO DE ESTUDO E LOCAL: Estudo transversal, hospital terciário do Estado de São Paulo. MÉTODOS: Oitenta e oito questionários distribuídos para pediatras e residentes em pediatria. RESULTADOS: Analisados 36 questionários. A maioria dos entrevistados (61,1 por cento) foi de residentes em pediatria e/ou neonatologia. Oitenta e três por cento realizavam alguma conduta especial com bebês de alto risco para surdez, e 55 por cento referiram não ter conhecimento sobre técnicas de triagem auditiva. A maioria desconhecia as classificações de grau e tipo de perda auditiva. Para 47,2 por cento, a criança poderia utilizar o aparelho auditivo a partir de seis meses. A maioria referiu que a criança poderia realizar reabilitação auditiva nos primeiros seis meses de vida e todos os entrevistados responderam que é função do médico ter preocupação com a comunicação da criança. CONCLUSÕES: Mesmo a maioria adotando condutas especiais com bebês de alto risco para surdez, não se investigava rotineiramente a audição. Todos os entrevistados julgaram ser função do médico se preocupar com a comunicação da criança.

OBJETIVO: Avaliar os conhecimentos sobre o uso e manejo de inaladores pressurizados dosimetrados entre profissionais de um hospital pediátrico terciário. MÉTODOS: Foram realizadas avaliações sobre o conhecimento do uso de inaladores pressurizados dosimetrados através de questionários teóricos e práticos, com médicos, fisioterapeutas, enfermeiras e auxiliares de enfermagem, atribuindo-se uma nota de 0 a 10 para cada avaliação. Calcularam-se as medianas das notas obtidas pelos profissionais de cada categoria, as questões com maiores e menores índices de erros, e foi feita a comparação descritiva do desempenho das diversas categorias profissionais. A análise estatística foi feita através do método de Kruskal-Wallis de comparação de medianas. Também foi realizada a análise através de regressão logística múltipla seqüencial. RESULTADOS: Foram avaliados 30 médicos residentes ou estagiários de pediatria, 23 médicos assistentes, 29 fisioterapeutas, 33 enfermeiras e 31 auxiliares de enfermagem. Os médicos residentes, fisioterapeutas e médicos assistentes obtiveram desempenho significativamente superior aos dos enfermeiros e auxiliares de enfermagem. Apenas os médicos residentes obtiveram mediana superior a 6. CONCLUSÕES: O conhecimento a respeito do uso de inaladores dosimetrados entre os profissionais de saúde da instituição é insatisfatório, principalmente entre enfermeiros e auxiliares de enfermagem, diretamente envolvidos na aplicação prática dos inaladores dosimetrados.


OBJECTIVE: To evaluate health professionals working at a tertiary pediatric hospital in terms of their knowledge regarding the practical use of metered-dose inhalers. METHODS: Practical and written tests on the use of metered-dose inhalers were applied to physicians, physical therapists, nurses and nursing assistants. A score from 0 to 10 was assigned to each evaluation, and median scores were calculated for each professional category. Questions with higher and lower correct values were identified, and a descriptive comparison was made regarding the performance of the various professional categories. Statistical analysis was performed using the Kruskal-Wallis method for comparison of medians. A sequential logistic multiple regression analysis was also performed. RESULTS: A total of 30 resident physicians or interns in the pediatrics department, 23 attending physicians, 29 physical therapists, 33 nurses and 31 nursing assistants were evaluated. Resident physicians, physical therapists and attending physicians performed significantly better than did nurses and nursing assistants. Only resident physicians had a median score greater than 6. CONCLUSIONS: The health professionals evaluated, particularly the nurses and nursing assistants, who are directly involved in the practical use of metered-dose inhalers, possess insufficient knowledge regarding the use of such inhalers.
